From helping with meals and mobility to personal care and errands, our in-home care lends a helping hand.

We offer a wide array of services that are tailored to every individual health needs. During the initial consultation with our professionals, your needs are discussed and determined for us to create a custom care plan that would cater and address your situation. Also, our care coordination team routinely visits and observes the patients and if necessary, modifies the care plan to meet their changing needs.

Skilled Nursing (SN)

Our trained nurses specialize in managing patient care in the home. Your nurse will perform a comprehensive evaluation and work closely with you and your doctor to develop an individualized plan of care that is in your best interest. During a nursing visit, a nurse will:

  • Check vital signs
  • Assess, clean, and dress wounds
  • Administer IV medications
  • Draw blood at home
  • Manage medications (Update medication lists and fill medi-sets)
  • Manage Coumadin
  • Provide education on disease processes and health concerns like diabetes, CHF, nutrition, hydration, high blood pressure
  • Toilet and Incontinence Care
  • Feeding and Special Diet

Physical Therapy

The goal of the Physical Therapy is to help you regain or improve your strength, balance, and coordination which in turn will help you to:

  • Safely get in and out of bed or a chair
  • Improve your posture and balance
  • safely use a cane or walkers
  • Learn about fall prevention

Occupational Therapy

The goal of the Occupation therapists is to help you regain the ability to perform everyday activities independently and to also help you learn skills that will help you cope with your disease process. Your Occupational Therapist will help you to:

  • Develop coping strategies for lost or reduced vision, hearing, or other senses
  • Teach you new ways to exercise or perform physical activities that account for limitations and to
  • be aware of the exacerbation of illness.
  • Live safely and comfortably in your own home

Home Health Aide

The Home Health Aide works under the supervision of the RN. Your Home Health Aide will assist with personal care to help make daily living a little bit easier. They use techniques for a safe transfer to chairs, beds, and couches. They also help with bathing, dressing and assist with bathroom needs. These are just a few of the services your Home Health Aides perform to help you live safely at home.

Personal Care Attendant

Wells HealthCare offers personal care attendant services also known as the GAFC program. This is a MassHealth program designed to serve clients living in affordable housing who are elderly and/or disabled. The goal is to provide support with activities of daily living (ADLs) and instrumental activities of daily living (IADLS) to eligible individuals so that they can continue living safely in their homes and community.

Assistance with activities of daily living (ADLS) to eligible individuals which include:

  • Personal Care
  • Bathing
  • Dressing
  • Grooming

Instrumental activities of daily living (IADLS) to eligible individuals which include:

  • Light Housekeeping
  • Grocery Shopping
  • Meal Preparation
  • Laundry
  • Medication Cueing
  • Transportation Coordination

Multidisciplinary Team Case Management

Multidisciplinary team case management to help you meet your goal of living as independent as you can. Our team consists of a registered nurse (RN) for wellness nursing visits and a medical social worker (MSW).

To be eligible for the GAFC program, you must meet the following criteria.

  • Must be 22 years of age or older
  • Must be Insured by MassHealth Standard or other specified Senior Care Options and One Care Plans
  • In need of daily assistance with personal care.
  • Approved by your physician and healthcare plan for personal care services.
  • Reside in “Clustered Housing” (i.e., subsidized apartment building).
