For many seniors, living at home for long as possible is a priority. But the natural aging process can make that difficult over time. At Wells HealthCare , we provide in-home care services that can help prolong independent living.

Our Interactive Caregiving approach transforms their day-to-day caregiving into opportunities for meaningful interactions. This includes conversation and activities that engage and enrich the physical, mental, social, and emotional lives of seniors. From helping with meals and mobility to personal care and errands, our in-home care lends a helping hand.

We offer a wide array of services that are tailored to every individual health needs. During the initial consultation with our professionals, your needs are discussed and determined for us to create a custom care plan that would cater and address your situation. Also, our care coordination team routinely visits and observes the patients and if necessary, modifies the care plan to meet their changing needs.

Group Adult Foster Care (GAFC)


Wells HealthCare offers group adult foster care also known as the GAFC program. This is a MassHealth program designed to serve clients living in affordable housing who are elderly and/or disabled. The goal is to provide support with activities of daily living (ADLs) and instrumental activities of daily living (IADLS) to eligible individuals so that they can continue living safely in their homes and community.

Assistance with activities of daily living (ADLS) to eligible individuals which include:

  • Personal Care
  • Bathing
  • Dressing
  • Grooming

Instrumental activities of daily living (IADLS) to eligible individuals which include:

  • Light Housekeeping
  • Grocery Shopping
  • Meal Preparation
  • Laundry
  • Medication Cueing
  • Transportation Coordination

Multidisciplinary Team Case Management


Multidisciplinary team case management to help you meet your goal of living as independent as you can. Our team consists of a registered nurse (RN) for wellness nursing visits and a medical social worker (MSW).

To be eligible for the GAFC program, you must meet the following criteria.

  • Must be 22 years of age or older
  • Must be Insured by MassHealth Standard or other specified Senior Care Options and One Care Plans
  • In need of daily assistance with personal care.
  • Approved by your physician and healthcare plan for personal care services.
  • Reside in “Clustered Housing” (i.e., subsidized apartment building).

What is Adult Foster Care?


Funded by MassHealth (Medicaid), Adult Forster Care allows a family member or friend to provide 24-hour care for qualified members in their homes. Members do not need to be bedridden to qualify. Unlike nursing homes or assisted living, Adult Foster Care allows the member to stay home. Qualified Caregivers receive a tax-free payment for their services, allowing them to provide 24-hour in-home care for their loved ones.

Adult Foster Care Services through MassHealth could be the solution for persons who require Active Supervision or Physical assistance with mobility, transferring, dressing, toileting, eating, and bathing

Eligibility Requirements

  • Must be 16 years and older.
  • Must have Mass Health Standard or CommonHealth for medical insurance.
  • Must require assistance with ADLs and IADLs daily
  • Has a medical condition or cognitive impairment that requires hands-on assistance or active supervision and coaching to complete ADLs
  • Requires supervision at least 21 hours per day.
AFC is a medical service that the member's PCP office must prescribe.

Caregivers


AFC Caregivers are thoroughly pre-screened and qualified as family member or a nonrelated individual. Caregivers Must be:

  • At least 18 Years of age
  • Must be responsible and capable of making precise decisions.
  • ADULT FOSTER CARE TEAM SERVICES
  • The AFC staff provides support, training, and coaching to members and caregivers.

Multidisciplinary team services include


  • Psycho-social or other applicable member assessments
  • Plan of care to identify all goals and objectives to maximize care.
  • Nursing and care management support
  • Individualized training and support
  • Monthly in-home visits by RN and Care Manager
  • Information about community resources and other local programs
  • 24 hours on-call system
