What is Adult Foster Care (AFC) and who qualifies?

Adult Foster Care is a MassHealth program that allows a family member or trusted caregiver to provide daily care to an eligible individual living at home. To qualify, the individual must be at least 16 years old, have active MassHealth Standard or CommonHealth coverage, need help with daily living activities, and have a caregiver available in the home.
